You can pay any required co-payment now or later.

It's Simple!

Give us your insurance information; member name, member ID number, group name, group number, coverage, and co-payments or deductibles.

• FirstSight will bill your insurance carrier for today's exam.
• If your vision plan pays us, great! Or if they pay you, send us the amount they paid you plus any co-payment necessary, and you're done.
• If your vision plan doesn't pay for the exam or reimburse you - FirstSight will send you a bill in about 60 days.
• This offer won't apply for future exams, if your insurance doesn't pay and you fail to pay as well.
